Staff Benefit Bundle**

The Staff Benefit Bundle includes a three-product bundle of Term Life/AD&D, Short Term Disability (STD) and Long Term Disability (LTD). Coverages are issued by The Prudential Insurance Company of America.

Eligibility

• W-2 employee of a licensed financial advisor/firm, working a minimum of 30 hours per week
• Active Advisor Staff Membership of the Financial Services Institute

Short-Term Disability

• You have two options at time of application
  • 14 Day Elimination Period covering 70% of Weekly Earnings
  • 29 Day Elimination Period coverage 60% of Weekly Earnings
• Maximum weekly benefit is $1,000

Long-Term Disability

• Elimination Period: 90 Days
• Monthly Benefit: 60% of weekly earnings, up to $10,000 Maximum^*
• Maximum Benefit Period: Normal Retirement Age under the Social Security Act

Life / AD&D Insurance

• $50,000 of Life Insurance and $50,000 Accidental Death & Dismemberment benefit
• Includes Accelerated Benefit Option ¹
